古罗马的元首制，是拉丁文Principatus（英文：Principate）此一政治制度的中文翻译。Principatus源自于Princeps——直译为第一公民。由这位第一公民来领导国家的制度，就称为元首制。在罗马史的划分上，罗马的“元首制”始于公元前27年，终止于三世纪的末期。

公元前一世纪的罗马共和国发生了连番的内战，最后由屋大维扫除群雄，建立不可动摇的政治地位。由于罗马共和在传统上，一向厌恶“国王”的称号，于是屋大维便在共和国的架构上，让第一公民同时拥有传统的保民官、统帅等统治大权，并时常参与执政官的竞选。基本上，元首制通常被认为是“披着共和制外衣”的君主制，而后代史学家通常用皇帝来指称这个时期的国家领导人，并将屋大维改制后的罗马国家视为罗马帝国的开端。

虽然元首制时代的罗马帝国领导人已经是一位终生执政的独裁者，而共和时期的象征——罗马元老院——对于第一公民并不具备必要的抗衡之力，但元首制时期的罗马皇帝仍需经过元老院的附议，法案才得以成立。并且当前任皇帝去世之后，即使国家群雄发起内战夺权，继任皇帝也必须经由元老院的认可，才算是法理上的罗马皇帝。公元69年的维斯帕先皇帝受军队拥立为帝之后，朱利亚·克劳狄王朝终结，罗马元老院便以共同决议的方式，明文订立了罗马皇帝的各项权力。从现今的铜版铭刻残本中，后人可以得知元首制下的皇权特征：

皇帝的权力基础系由元老院和人民的授予，而非天授。
皇帝没有整体与无所不包的权力，系由若干个别的执权，如行政、立法、外交、任免大权的集合，包括人民对其所生的威望。
皇帝必须受到法律的约束，亦即“法律超越于皇帝之上（Leges super principem）”。

在罗马帝国时代，能够成功施行元首制的皇帝，首推安敦尼王朝的五贤帝时期，皇权与上层阶级合作，让国政推行顺畅。而在尼禄、图密善时期，则是因为这两股势力彼此互相仇视，造成所谓的恐怖统治与政治骚动。

到了罗马帝国的三世纪危机时期，元首制无论在实质或形式上，皆无法再维持旧的政治面貌。直到皇帝戴克里先执政，毋需元老院背书的“皇帝敕令”已成为帝国行政的方式，元老院退化成皇帝的“咨询机构”，使得元首制结束。
