Mandibular ikinci küçük azı dişi, mandibular birinci premoların distalinde (orta hattın uzağında) ve mandibular birinci moların mezialinde (orta hattın yakınında) konumlanmıştır. Bu premoların görevi çiğneme esnasında mandibular birinci molara yardım etmektir. Mandibular ikinci premoların üç adet tüberkülü vardır. Bukkalde (yanağa bakan tarafta) bir büyük tüberkül vardır ve lingualdeki (dile bakan tarafta) iki tüberkül iyi gelişmiş ve fonksiyoneldir, bu bakımdan çiğnemeye yardımcıdır. Mandibular birinci premolar küçük bir kanine benzerken mandibular ikinci premolar daha çok bir birinci molara benzemektedir. Mandibular premolarlar süt dentisyonda bulunmazlar. Süt dentisyonda daimi premolarların yerini tutan dişler premolar değil süt molar dişlerdir.
