《头号书迷》（英语：Misery）是一本由史蒂芬·金1987年出版的恐怖小说，内容描述作家与读者之间钩心斗角的心理战，该书在1990年曾改拍成电影《头号书迷》，其中饰演女主角安妮的凯西·贝兹亦获得奥斯卡最佳女主角奖。
1987年，曾以《惨不忍睹》为名由皇冠出版社出版。1990年随着电影的上映，皇冠重出了《头号书迷》。两者皆绝版之后，远流出版社于2006年7月1日重新出版。
刚刚完成自认生平最棒小说《快车》的《苦儿》系列畅销作家保罗·薛顿（Paul Sheldon），驾着跑车，打算好好放松去度假，不料却遇上一场大风雪，加上酒精影响而遇难，最后由一位中年妇女安妮·维克斯（Annie Wilkes）救起。安妮是保罗《苦儿》系列的头号书迷，然而保罗却对安妮有着更深的恐惧。
安妮乍闻保罗将系列主角苦儿·雀斯汀（Misery Chastain）赐死后，要求保罗写一本《苦儿还魂记》。安妮以药物控制保罗，保罗若是不从，安妮便以药物威胁。保罗趁安妮外出时苦思逃脱的方法，然而遭安妮发现后，遭其用斧头废其左脚掌与大拇指。保罗身心受创，打算以《苦儿还魂记》的完稿日与安妮一决生死。
《苦儿还魂记》完成后，保罗千方百计终于杀死了安妮，最后得到两名警员相救。虽然保罗而后顺利救回，《苦儿还魂记》也顺利出版，但是保罗心理对于安妮始终有着挥之不去的阴霾与恶梦。
