I took out all the footnotes because practically none of them were any use: they were brief references to books that weren't in the bibliography, links to disappeared web pages, etc. Luckily, they were not supporting anything controversial. The one that was really useful was a reference to the excellent article in the Stanford Encyclopedia of Philosophy. This article is of general relevance, so I put it in the bibliography.
